Problematic Social Media Use Linked to Rising ADHD Symptoms in Teens

Teenagers who reported unusually high levels of hard-to-control social media use tended to show worsening attention and hyperactivity symptoms in the following years, according to a study of more than 11,000 US adolescents published this week in JAMA Network Open. The effect was consistent for boys but weaker and less reliable for girls, and the researchers were careful to note that the influence, while measurable, was modest.

The finding sharpens a long-running debate. Much of the alarm over screens and young minds has focused on sheer time spent online. This study instead zeroes in on a specific, more troubling pattern — use that carries the hallmarks of addiction, such as an inability to cut back — and asks whether it precedes changes in attention rather than merely coinciding with them.

How the study was designed

The researchers, led by Elizabeth J. Li of the University of California, San Francisco's Division of Adolescent and Young Adult Medicine, drew on the Adolescent Brain Cognitive Development Study, or ABCD — the largest long-term study of brain development and child health in the United States. They followed 11,286 youths across five annual assessments, from roughly age 12 through mid-adolescence, spanning 2017 to 2023. About 52% of the sample was male.

Two measures anchored the analysis. Problematic social media use was captured through the youths' own responses to the Social Media Addiction Questionnaire, which probes compulsive patterns like preoccupation and failed attempts to reduce use. ADHD symptoms were rated separately by parents using the attention-problems scale of the widely used Child Behavior Checklist — a design that avoids having the same person report both the behavior and its supposed consequence.

To untangle cause from correlation, the team used a statistical approach called a random-intercept cross-lagged panel model. Its purpose is to separate stable, between-person differences — some kids simply use social media more and have more attention problems than others — from within-person change over time. That within-person lens is what lets researchers ask whether a given teenager's symptoms rose after their own social media use climbed above its usual level.

What the data showed

At the within-person level, years in which a teen's problematic use ran higher than normal were followed by small but statistically reliable increases in parent-reported ADHD symptoms. From year 4 to year 5 and again from year 5 to year 6, the lagged association held at a standardized coefficient of 0.08 — a modest effect by conventional standards, but a directional signal that survived the study's stringent modeling.

The reverse pathway was much fainter. Higher ADHD symptoms predicted a slight uptick in later problematic use in only one interval, and the effect was small and sporadic. In other words, the data leaned toward problematic use nudging attention symptoms upward more than the other way around — though the study, being observational, cannot prove that social media use directly causes the change.

The sex difference was among the most striking results. The prospective link from problematic use to later ADHD symptoms was significantly stronger in boys than in girls. The authors did not settle why, but the pattern echoes other research suggesting adolescent boys may be more susceptible to the compulsive design features of games and social platforms.

Why the distinction matters

ADHD is now one of the most common neurodevelopmental diagnoses in American childhood. Roughly 7 million US youths — about 11% of those aged 3 to 17 — have been diagnosed at some point, a figure that rose by a million between 2016 and 2022. Against that backdrop, any modifiable contributor to attention problems draws attention itself.

The study's contribution is less about magnitude than about specificity. Several large cohort analyses have found little evidence that raw screen time causes ADHD, and this research does not overturn that. What it isolates is the problematic quality of use — the addictive pattern — as the part worth watching. For clinicians and families thinking about co-occurring mental health concerns, that reframing points screening toward how a young person uses social media, not just how long.

Read the effect sizes honestly

The authors are explicit that the population-level associations are small, and a coefficient of 0.08 will not, on its own, explain a child's attention difficulties. Because the study is observational, unmeasured factors — sleep loss, family stress, or an underlying vulnerability that drives both behaviors — could contribute to the pattern. The reliance on a self-reported addiction questionnaire also introduces the usual limits of adolescents appraising their own habits.

Even so, the design is unusually rigorous for this contested field, and its practical takeaway is measured rather than sweeping. Screening adolescents for hard-to-control social media use, paying attention to sex-specific risk, and treating compulsive patterns as a distinct signal from ordinary screen time may be warranted. The study stops well short of a mandate to pull teenagers offline; what it offers is a sharper question for parents and clinicians to ask about the quality, not just the quantity, of a young person's time on their phone.
